Please also download this Health Information Form and ask for your Doctor to complete, before sending back to us at either our men’s centre Sunnybrae or women’s centre Benaiah.
3. Phone In
We ask all applicants to phone in once a day (including weekends). If no one is available to take your call, then please leave a message on the answer machine. You can call this number 24/7.
This helps us to see how motivated an individual is and enables us prioritise the waiting list. Phoning in every day helps the applicant have some focus for the day and support and encouragement if needed.
